The Roux-en-Y Gastric Bypass
The Laparoscopic Roux-en-Y Gastric Bypass is an operation that has gained wide acceptance in the past decade, especially as laparoscopic versions have been introduced. Gastric BypassThe Laparoscopic Gastric Bypass has a weight loss potential in the range of 70-75% of the excess body weight, on average, and the weight loss is fairly well sustained. The Roux-en-Y Gastric Bypass does, in most studies, usually show some mild regain of weight at five years post-surgery.

The Laparoscopic Gastric Bypass makes a small pouch at the top of the stomach, and then routes the food through a "Roux" limb of small bowel before it joins the biliopancreatic secretions.

The Laparoscopic Gastric Bypass is the most commonly done laparoscopic bariatric procedure in the United States. The Gastric Bypass is good for patients who have a moderate to large amount of weight to lose.

The Laparoscopic Sleeve Gastrectomy with Duodenal Switch (DS)

Duodenal Switch The Laparoscopic Sleeve Gastrectomy with Duodenal Switch procedure, sometimes known as the Biliopancreatic Diversion with Duodenal Switch, combines elements of a restrictive operation with malabsorption of fat to give you a very powerful bariatric surgery procedure. You may have heard of the Duodenal Switch procedure referred to as the BPD/DS, the "Duodenal Switch", the "DS", or simply the "Switch".

Dr. Kular performs the Totally Laparoscopic Sleeve Gastrectomy with Duodenal Switch, with a Hand-Sewn Duodenal-Ileal Anastomosis. He is the fifth bariatric surgeon in the world to perform the totally laparoscopic Duodenal Switch procedure by the Hand-Sewn Duodenal-Ileal Anastomosis Technique of the Laparoscopic Duodenal Switch. There are few bariatric surgeons who perform the Totally Laparoscopic Sleeve Gastrectomy with Duodenal Switch procedure.

The Laparoscopic Sleeve Gastrectomy

Sleeve Gastrectomy The laparoscopic sleeve gastrectomy is a relatively new operation that can be done either as a standalone procedure for those who don't have much weight to lose, or as part of a staged operation for high risk patients. The weight loss is projected to be in the range of 45 to 55% of the excess body weight, which is comparable to that of the Laparoscopic Adjustable Gastric Banding Procedures, but it doesn't have the foreign body problems such as erosion or slippage that can occur with those procedures.

Weight Loss Mechanism
The reduction of the size of the stomach, to about 100 - 120 cc in volume results in a restrictive weight loss. As a result, patients feel full after a very small amount of food, and therefore lose weight because they eat less, and they are happy eating less.

Preservation of Pyloric Valve
Also very important is the fact that it preserves the pylorus, the valve that regulates emptying of the stomach. This acts as "natures band" and allows food to hold up in the stomach for a while, making the person feel full while the food trickles out. Coupled with the fact that there is no rearrangement of the bowel, it also means dumping and marginal ulcers are not a problem. The normal satiety mechanism is preserved.

The Operation
The operation is done with 5 small incisions, and takes about an hour to do. We do a "leak test" in the operating room before we complete the operation. Having the Laparoscopic Sleeve Gastrectomy involves an overnight stay in the hospital. There is no drain or nasogastric tube. You are able to return to work, resume heavy lifting and strenuous activity, in most cases, in about two weeks from the time of surgery. If you are able to do light duty at work, there is the possibility of going back to work sooner than two weeks for some patients.


LAP-BAND®

The LAP-BAND  is a prosthetic device that wraps around the upper part of the stomach to partition a small upper stomach pouch, restricting its outlet and thereby inducing a weight loss through decreased intake of food. The small pouch above the LAP-BAND acts to induce a feeling of satisfaction after only a small amount of food.

LAP-BAND<sup>®</sup>The LAP-BAND system produces a weight loss in the range of 50-55% of the Excess Body Weight. The results with the LAP-BAND system aren't as good, generally speaking, as with the Laparoscopic Roux-en-Y Gastric Bypass or the Laparoscopic Duodenal Switch, but the risks involved are less. The LAP-BAND system could be converted into a Laparoscopic Duodenal Switch if it fails. For certain patients, the LAP-BAND  system can be a very effective operation.

The LAP-BAND  adjustable gastric banding device is placed completely laparoscopically.
